Welcome to the Each One Teach One Learning Center family! We are thrilled to partner with you in your child’s growth and learning journey. Our mission is to provide a safe, loving, and enriching environment where every child’s unique gifts are celebrated and supported. We believe children learn best when they feel seen, valued, and encouraged, and our goal is to prepare each child for kindergarten with confidence and curiosity.
To complete enrollment, please use the link below to fill out the four required state forms. These forms are necessary for health, safety, and nutrition compliance and help us serve your family better.

Please select the language that works best for your family and complete only one version of each form.

2. CACFP Income Eligibility Form (MO 580-1314)
3. Infant & Toddler Feeding and Care Plan (MO 500-3306)
After submitting your forms:
• A link to download your completed forms will appear on the “Thank You” page.
• Please save or print a copy for your records.
• The medical form must be taken to your child’s doctor so they can complete their portion.
